Re-ablement service

The Winchester Reablement Service

Based in Nightingale Wing, Therapy Services at Royal Hampshire County Hospital, the Re-ablement Service aims to provide a multi-disciplinary team approach to assessment and rehabilitation of people with health needs who live within Winchester and the surrounding area, thereby preventing hospital admission and supporting sustainable discharges.

Services include:

  • A streamlined multi professional falls service from prevention to inpatient follow up.
  • Community based falls prevention/balance classes.
  • Specialist Neurological rehabilitation and tone management.
  • Links with local authority leisure centre based exercise classes to offer on going maintenance of health and functional gains.
  • Stroke follow up reviews and rehabilitation.
  • Specialist amputee rehabilitation facilitating earlier discharge from hospital and optimised functional recovery.
  • Patient and carer support, including education groups for people with a new or recent diagnosis of Parkinson’s.
  • Exercise classes offering health promotion, balance work, gait re-education, falls prevention/management, increasing confidence levels, stamina, flexibility and muscle strengthening.
  • Multi-disciplinary community rehabilitation (Physiotherapy, Occupational Therapy, Nurses and rehabilitation assistants).
  • Access to a multi-disciplinary team consisting of physiotherapists, occupational therapists, nurses and rehabilitation assistants for rehabilitation and advice tailored to suit individual need.

The Team

Managed by the Re-ablement Service Team Leader, the team comprises of physiotherapists, occupational therapists, nurses, rehabilitation assistants, generic healthcare worker and GP with a specialist interest in falls.

Referral Criteria

The service addresses the health needs of people who are likely to benefit from a holistic rehabilitation programme.

Referral Process

Referral is open to all health care professionals and should be made directly to the service by telephone, post or fax.  It is not necessary to refer to the consultants in elderly care or for the referral to come through the patient’s GP.

Service Delivery

Rehabilitation will take place either at home, within the Re-ablement Service, at work or within an NHS location depending on need and patient choice.
