Re-ablement service

We are multi-professional team providing assessment and treatment for patients with specialist rehabilitation needs either at home, work or within an NHS location depending on need and patient choice. We are based within Therapy Services at Royal Hampshire County Hospital. Detailed below is the range of services we can offer to those in our catchment area (please contact the service on the above number for details about this).

This is not a rapid response service: all referrals are prioritized on receipt.  Rehabilitation periods are goal orientated and carried out in the environment most appropriate to meet patient needs.

Falls Clinics

Multi-disciplinary falls assessment for patients who have a history of falls or are at high risk of falls.  This is a 2-3 hour long assessment where patients are assessed by an occupational therapist, physiotherapist, nurse and GPSI.  This is a one off assessment, and patients are then signposted into services most appropriate to suit their needs/outcomes of the assessment.

Amputee Clinic

Multi-disciplinary rehabilitation clinic for patients following a lower limb amputation.  Clinic is staffed by physiotherapy, occupational therapy, nurses and assistants.  Patients attend twice weekly or as agreed with therapists.

Outpatient Neurological Physiotherapy

The neurology physiotherapy outpatient aims to give specialist neurology assessment and treatment to adult patients with a neurological condition.  We accept referrals for patients with conditions such as stroke, multiple sclerosis, Parkinson's disease, transverse myelitis, post polio syndrome, dystonia, motor neurone disease, and many other rare neurological conditions. 

Specialist neurology physiotherapy aims to progress patient's balance, gait, and strength and can assist efficient and good quality movement after a neurological event.

Parkinson’s Education Group

This is an education group for individuals with a recent or new diagnosis of Parkinson’s, and their carers/families/friends.  These individuals are given specific education talks by a multi-disciplinary team of professionals and the service aims to promote and encourage self management of the disease.  Staff groups involved include: Parkinson’s Nurse Specialist, Speech and Language Therapy, Dietetics, Occupational Therapy, Physiotherapy, Clinical Psychology.  A Research Therapist from the local research network and the information support worker from Parkinson’s UK also attend one of the sessions.

Better Balance Group

This is a 12 week progressive course for physically active/able individuals who have balance difficulties.  The classes target stamina, balance, strength and flexibility.  Patients must have reasonable sight, hearing and be able to get up from the floor with minimal assistance to be able to attend this group.  The classes also include health promotion and advice.

Re-ablement Exercise Class

This is an exercise class for people with reduced mobility, balance problems, reduced strength, stamina or confidence.  The class includes chair based and standing exercises, which are adapted to meet the individual needs of our patients.  Exercises aim to improve mobility, gait, balance, strength, stamina, confidence and functional abilities.  Patients are assessed by the multi-disciplinary team of nurses, occupational therapy and physiotherapy.  Patient centred goals are agreed with patients on their first attendance.  Home visits are available if required.

Multi-disciplinary Community Rehabilitation

This multi-professional community rehabilitation service is available to clients, who require community based rehabilitation.  Depending upon need, patients may be visited at home by one or more of the Re-ablement Team, (Occupational Therapists, Physiotherapists, Nurses or Therapy Assistants/Technicians). Patients referred to this service may be experiencing problems with activities either at home or in the community.  These activities may be more difficult due to ill-health or disability, or perhaps due to lack of confidence.
