Clifton ward

Clifton Ward is a 26 bedded unit which specialises in rehabilitation of the older person. Our focus is on providing a positive atmosphere where older people can achieve their potential, are supported to become more independent and have the best discharge outcome possible.
We are able to do this with a multidisciplinary team (MDT) approach involving Doctors, Nurses, Physiotherapists, Occupational therapists and an Activities Co-ordinator. There is also involvement when required from Speech and Language specialists, the Diabetic team, Dieticians, Mental Health specialists and the Dementia team. We are working very hard to ensure that each individual is discharged as quickly and smoothly as possible and to accomplish we can work closely with social workers and the ward based multidisciplinary facilitator. Together these individuals will be planning from the moment the patient arrives on Clifton to achieve the best discharge outcome possible with the patient and their family. The team is supported by a ward clerk, housekeeper, domestic staff and we also benefit from volunteers that visit and assist at mealtimes and with talking therapies.
Clifton ward facilities consist of a gym with walking bars, practise stairs and a therapy couch. This area is also used by patients for coffee morning and tea time activities. Patients are encouraged to take part in arts, crafts and games with our activities co-ordinators. We are currently raising funds to develop a courtyard area so that outside time can be enjoyed by all.
We welcome and value the input of family and friends whilst planning care and recognise that our success depends on close co-operation and good two way communication with all involved. You can always speak to the trained nurse who is caring for your relative and it can also be arranged for you to speak with Doctors on the ward when possible. Please raise any concerns that need addressing as soon as possible to ensure that they can be dealt with as quickly as possible.

Useful Information

To aid rehabilitation it would help the patients to have the following items:
Comfortable loose fitting clothing, nightwear, supportive footwear and any toiletries that would normally be used. It is also important that patients have their glasses, dentures and hearing aids with them. It is best that large amounts of money and valuable items are left at home as we are unable to take responsibility for them.

Visiting and Meal Times

Visiting
3pm till 8pm

All inpatient wards at Hampshire Hospitals have adopted John’s Campaign, which allows the carers of patients with dementia to stay in hospital with their loved ones if they wish.

Mealtimes
Breakfast - 8.30am til 9.15am
Lunch - 12.30pm till 13.15pm
Dinner - 17.30pm till 18.15pm

Clifton ward operates protected mealtimes. We try to make the ward quiet and calm to enable patients to concentrate on eating without distraction or interruption. Family members wishing to participate at mealtimes are welcome to visit at these times
