Pre-Assessment

If you are due to come in to hospital for an operation or procedure you may require a pre assessment before your admission.

If you do require a pre assessment you will be advised at your outpatients appointment and given information which will enable you to start completing your assessment from home using Synopsis.

Please do not worry if you are not given the information about Synopsis as it may not be necessary for your procedure.

  • We work with the Multi-Disciplinary Team (MDT) to ensure that you are safe and medically fit for the anaesthetic 
  • Base line observations (including blood pressure, height and weight) screening for  MRSA/MSSA , ECG, bloods and fasting instructions will be provided

  • Please complete the questionnaire prior to your appointment and bring the details of you surgery date and time.
  • Allow yourself parking time or use Park and Ride
  • Be punctual for your appointment, preferably be 10 min early to have your BP, ECG and BMI check.
  • Please feel free to bring in additional support if you feel that you needed.
  • If you have had investigations or have pending investigation in other hospitals than Royal Hampshire County Hospital (RHCH) please bring in any documentation with you.

As part of the preparation for your surgery it is essential that you take very good care of your skin.  You need to be extremely careful not to sustain any bites, grazes, abrasions or cuts in the weeks leading up to your surgery, this is not just on the area being operated on but over your whole body.

Any open area on your skin is an area which could potentially become infected. If we proceed with your surgery then your new wound could easily become infected which could prevent the wound healing as well as causing you distress and discomfort.
 

You need to be aware that the surgeon WILL CANCEL YOUR SURGERY if you have any bites, cuts, abrasions or grazes. It is therefore very important that you contact the pre-assessment clinic and talk to the staff as soon as possible before your surgery, so that we can give you advice or arrange to see you and assess your skin.

If you have any coughs, colds, Flu symptoms, Stomach bugs or infections within 10 days of surgery please also contact us for advice.

  • Synopsis Home is a secure web -based programme which allows you to complete your pre-operative health questionnaire online. 
  • All patients must complete a pre-operative questionnaire for surgery to go ahead. It is to assess your fitness for surgery.
     

  • Synopsis Home is requested because you need a procedure or an operation. All patients having an operation need to have a pr-operative assessment, where we assess your fitness for surgery.
  • Completing Synopsis Home allows the pre-assessment nursing team to review and decide which tests you may need prior to your operation. Completing it quickly and answering all the questions will help prevent delays in your surgery.
  • The questionnaire is available for you to complete as soon as you have finished your face–to-face consultation in the Outpatient Department. You will be given a letter which the details of how to log on to the online form from home to enable you to complete the questionnaire. If you have no computer access or struggle with computers then the outpatient team will be able to assist you in filling out the questionnaire; with designated devices after your consultation or you can complete this at home.
  • If you had a telephone or video appointment instead, you will receive a text message with a link to complete Synopsis Home within a few days of your appointment having taken place.
     

It is easy to self-register to Synopsis Home, you simply go to the website www.synopsishome.com/login

  • Register by filling in your name and NHS number. 
  • This will give you access to the health questionnaire. This will take about 15 minutes to complete. You will need your height, weight, blood pressure (if you have access to check it) and the name and dose of all medication you currently take. 
  • You can then choose to complete the questionnaire at a time that is convenient for you - for example in the comfort of your own home. Please answer all the questions as completely as possible. 
  • Once completed, the questionnaire is shared directly with the hospital. Your data can only be viewed by the hospital.

This will be on the top of your appointment letter from the hospital or you can call your GP to obtain it.

  • You will receive a confirmation email once your healthcare questionnaire has been completed and submitted. 
  • You have nothing more to do. 
  • Once the pre-assessment nurses triage the questionnaire you will either be added to the waiting list or they may contact you for further information or investigations.
