The Children’s Home Acute Team (CHAT) is a team of experienced paediatric nurses who provide assessment and support to children at home, to enable them to be cared for whilst they are unwell.
Our aim is to reduce unnecessary visits and/or admission to hospital for the child. We do this by offering telephone support and home visits. If further medical support is needed, this is discussed with the paediatric doctors. Support is provided for the child and their family until they are recovered from their illness.

We receive referrals from GP practices in North and Mid Hampshire, Basingstoke Hospital, Winchester Hospital and the emergency departments at both sites (A&E).
We only accept referral with a diagnosis of one of the following conditions:
- Bronchiolitis (viral lung infection)
- Chest Infection
- Croup (respiratory infection)
- Gastroenteritis (diarrhoea and/or vomiting)
- Tonsillitis (inflammation of the tonsils)
- Viral-induced wheeze
- Viral upper respiratory tract infection (URTI)
- Suspected or positive COVID-19 (coronavirus) infection
- UTI
- Otitis Media
- Chicken Pox
- Hand Foot and Mouth
- We also Undertake prolonged jaundice screen at homme for new babies
