Patient Safety Incident Response Framework
From 1 October 2023, Hampshire Hopsitals has implemented the new national Patient Safety Incident Response Framework (PSIRF).
This replaces the current serious incident response framework with a focus on learning and improvement, involving those affected and a compassionate response.
Learning from deaths
Our Foundation Trust is committed to ensuring that there are robust processes in place for identifying, reporting, investigating and learning from deaths.
The Foundation Trust follows national guidance is this area and the process is outline in the Trust Learning from Death Policy.
