Thank you for taking part in an interview/ photography/ filming today.

As will have been discussed with you, this is being used to publicise our services and celebrate the great things happening here at HHFT. This may include internal (i.e. staff Facebook) and external (public website) communications and be physical or digital. If you have any questions please email communications@hhft.nhs.uk.

Your name/ quote/ image may be used both now and in the future across print and digital platforms including on our website and social media, bedside media units, printed publications, in the media, posters and leaflets. If you were filmed or interview by a third party (i.e. the BBC or ITV) they will own the content rather than the trust. They are also responsible for its use/ broadcast.

If at any time you wish to withdraw your consent and/ or no longer want to be featured in HHFT communications, please email communications@hhft.nhs.uk with the subject “Withdrawal of consent”. Please include a clear description of the activity you were part of and when it took place – you do not need to explain why you wish to withdraw consent. We will immediately remove your interview/ image from all HHFT sites and channels (such as HHFT twitter) that we control. However, we will not be able to do so for channels or sites we do not control or if the content belongs to a third party.

Thank you again for taking part, it is very much appreciated.

Should a major incident occur the trust has a contingency plan that includes arrangements for keeping the media, staff and patients informed. A dedicated media area will be established at either Andover War Memorial Hospital, Basingstoke and North Hampshire Hospital or Royal Hampshire County Hospital, and media will be informed where this is. The communications team will give regular updates on the situation. We will manage requests and opportunities for photographs, filming and interview requests.

The membership office sits with the communications team who co-ordinate all membership activity including regular updates and events for members and ensuring the membership database is representative of the local population the Trust cares for. To contact the membership office email communications@hhft.nhs.uk, telephone 01256 852615 or fill in our membership form online to become a member.
