Delivery of this strategy will take a one team approach, driven by our staff, shaped through co-production with patients, families and carers, and strengthened by the support of our partners. Together, we will work with clear priorities and shared accountability to achieve our objectives.

Our vision is clear, but we will continue to learn, listen and adapt. As needs, priorities and guidance change, we will update our plans to ensure we deliver the best possible outcomes for patients, communities and staff.

Our delivery approach will be built on:

A five-year strategic plan
  • This is a rolling five-year plan that sets out the priorities and actions needed to deliver our strategy and respond to the 10 Year Health Plan.
  • We will review and update our five-year plan annually to ensure we remain responsive, focused and aligned with our ambition.
Co-production and partnership

• We will work together with patients, families, carers and staff to help shape and design service improvements.
• We will listen to what matters most to the people who use and deliver our services, ensuring their experience informs how care is provided and developed.
• Strong partnerships across Hampshire and the Isle of Wight will support more joined-up care and shared solutions to common challenges.

Alignment across the organisation

• We will drive ahead transformation programmes that bring together our major priorities for change.
• Individual objectives and team plans will connect every person’s work to our strategic priorities.
• There will be clear line of sight between our day-to-day activity and the outcomes we want to achieve for patients and communities.

Strong governance

• Oversight and assurance will be provided through our Board and governance structures.
• There will be clear accountability for delivery, transparency in decision-making and regular reporting on progress.
