Our ambition

Care will be designed around people andcommunities, not buildings or organisational
boundaries. Working with partners across Hampshire and the Isle of Wight, including
hospitals, GPs, community and mental health services, local authorities and the voluntary sector, we will deliver more joined-up care closer to home, with hospital care available when it is needed most.

Reducing health inequalities will be at the heart of our approach. We will work across organisational boundaries to target support where it is needed most, improve outcomes for people who face the greatest challenges accessing care, and help more people live healthier lives.

Technology will make it easier for people to access services, manage their care and stay connected with healthcare professionals in ways that better fit around their lives. Through innovation and the safe use of robotics, automation and artificial intelligence, we will make care more personalised, effective and responsive to people’s needs, helping them achieve better outcomes and experiences.

Building on our strengths in acute and specialist care, we will continue to develop high-quality, evidence-based services for patients with complex needs through clinical excellence, innovation and research.

By 2031, more people will stay healthier for longer, receive support earlier and be better able to manage their own health and conditions. Fewer people will need unplanned visits to, or stays in, hospital, and more will enjoy a better quality of life.

Measuring our success

We will measure our success through the difference we make to outcomes,
experience and access to care. This includes reducing emergency admissions,
diagnosing more cancers at an earlier stage, improving access to planned care,
maintaining high levels of patient satisfaction, and ensuring safe, high-quality
care with reduced avoidable harm. The full set of measures associated with this
ambition can be found here

Objective 1: We will improve population health and outcomes

By 2031, Hampshire Hospitals will be a Trust that plays a leading role in helping people across Mid and North Hampshire live healthier, more independent lives. We will be known for care that is easier to access, more joined-up and designed around people’s needs, with stronger support for those who face the greatest barriers to good health. Patients will have better access to information, advice and support, so they can take a more active role in managing their health and making decisions about their care.

  • Working with patients, families, carers and communities to design services, involving people in decisions about their care, including at the end of life, and using insight to understand what matters most, ensuring care is inclusive, accessible and focused on people's needs.

  • Tailoring our engagement so that children, young people and families can meaningfully influence how we design, deliver and improve our services.

  • Identifying health risks earlier, improving early diagnosis, supporting people to manage long-term conditions and providing timely access to care and treatment, helping people stay healthier for longer.

  • Improving access to information, advice, support and digital services, including a single patient portal or virtual front door, and expanding virtual health services with our partners so, where appropriate, patients can receive advice and monitoring from healthcare professionals remotely, without always needing to come into hospital. This will make care easier to access and enable more people to take an active role in managing their health and care.

  • Helping people get the right tests, treatment and support sooner, reducing delays, unnecessary appointments and avoidable hospital stays, and improving outcomes and patient experience.

  • Working seamlessly with partners across organisational boundaries, sharing information appropriately and targeting support where it is needed most, so we can offer care where it will make the most difference to patients’ outcomes, often outside the walls of our hospitals.

  • Promoting prevention, supporting healthier lifestyles and making every contact count, improving health and wellbeing and helping more people live healthier, more independent lives.

  • Targeting support where it is needed most and focusing on those who face the greatest barriers to accessing care, reducing health inequalities and improving outcomes for all.

  • Always considering social value, using NHS resources to support local people, communities and the environment and creating employment and development opportunities through partnerships with local organisations and schools.

Objective 2: We will evolve acute and specialist care

By 2031, we will be recognised as a leading provider of acute and specialist care.  More people will receive high-quality specialist care and benefit from expertise, innovation and clinical excellence. Through research, technology and strong partnerships, we will improve outcomes and help shape the future of healthcare.

  • Building on our strengths in acute and specialist care, developing centres of excellence and bringing together expert clinical teams so patients with the most complex needs receive high-quality care and the best possible outcomes. Our provider collaborative will increasingly be the place where we find solutions and ensure residents across Mid and North Hampshire receive the highest quality services with the best outcomes.

  • Strengthening specialist services and sharing expertise, knowledge and innovation through partnerships and specialist networks, so more people benefit from high-quality specialist care wherever they live, including beyond our hospitals.

  • Driving research, innovation and the adoption of new techniques and technologies, including robotic-assisted surgery, less invasive treatments and approaches that help people recover more quickly and achieve better outcomes.

  • Expanding diagnostics, straight-to-test pathways and evidence-based approaches to care, enabling earlier diagnosis, faster treatment and improved patient experience.

  • Working with commercial organisations, academic institutions and life sciences partners to apply research, insight, best practice and new learning, accelerating innovation and shaping the future of healthcare. We will grow the research activity across the Trust and alongside our partners.

  • Leading through collaboration, research, innovation and clinical excellence, strengthening our reputation as a leading provider of acute and specialist care and improving outcomes for the communities we serve.

Objective 3: We will deliver integrated neighbourhood-based care

By 2031, Hampshire Hospitals will become a trust where neighbourhood-based care is the default model, with more people receiving timely, joined-up care closer to home, whether at their GP surgery, in community clinics or at home. Services will work seamlessly together around people's needs, reducing delays, improving access to support and ensuring excellent hospital care is available whenever it is needed.

  • Stabilising our more vulnerable services, such as services that are difficult to recruit to, and strengthening partnerships across organisational boundaries to create more joined-up pathways across hospital, community and primary care.

  • Consistently meeting constitutional standards, ensuring timely access to assessment, treatment and support, across emergency, outpatient, specialist and planned care so people get the care they need more quickly.

  • Expanding our virtual wards and community diagnostic services whilst becoming a more active partner in neighbourhood hubs so people can receive timely care closer to home with fewer unnecessary hospital visits and a better experience of care.

  • Developing Andover War Memorial Hospital as a community asset, building on the strength of multi-agency working so it becomes more than just a place for healthcare, offering wider social value to the community.

  • Designing care around people's needs, preferences and circumstances, with services working together more effectively to provide seamless support and fewer delays.

  • Increasing the use of digital technology where it makes care easier, more convenient and accessible for patients.

  • Providing excellent emergency, inpatient, specialist and complex hospital care whenever hospital treatment is needed.
